Introduction
Ember Psychology Clinic ("we", "us", "our") is committed to protecting the privacy and confidentiality of your personal information. This policy outlines how we collect, use, store, and disclose your information in accordance with the Australian Privacy Principles under the Privacy Act 1988 (Cth) and the Health Records and Information Privacy Act 2002 (NSW).
Information we collect
We may collect personal information including your name, contact details, date of birth, Medicare number, referral information, and health information relevant to the psychological services we provide. We collect this information directly from you, from your referring GP or other health professionals, and through our practice management system (Halaxy).
How we use your information
Your personal information is used to provide psychological services, manage appointments and billing, communicate with your referring GP or other treating professionals (with your consent), process Medicare rebates, and meet our legal and professional obligations.
Storage and security
Your records are stored securely using Halaxy, a cloud-based practice management system that meets Australian health data security standards. We take reasonable steps to protect your information from misuse, interference, loss, unauthorised access, modification, or disclosure.
Sharing your information
We will not share your personal or health information without your consent, except where required by law (for example, mandatory reporting obligations). With your written consent, we may share relevant clinical information with your GP, psychiatrist, or other treating professionals involved in your care.
Telehealth
When sessions are conducted via telehealth, we use secure, encrypted platforms to protect your privacy. The same confidentiality standards apply to telehealth sessions as to in-person sessions.
Your rights
You have the right to access the personal information we hold about you, request corrections to your information, and withdraw consent for us to share your information at any time. To make a request, please contact us at emma@ember-psychology.com
Complaints
If you believe your privacy has been breached, please contact us directly. You may also lodge a complaint with the Office of the Australian Information Commissioner (OAIC) at www.oaic.gov.au or the Health Care Complaints Commission (HCCC) at www.hccc.nsw.gov.au.
